CONTRACT FOR THE PROVISION OF TRUSTED ASSESSOR SERVICE - PROVIDED BY INDEPENDENT TRANSFER OF CARE COORDINATORS

Tender description

Cheshireeast council in conjunction with CCG is seeking a provider to deliver the trusted assessment model in order to support the timely transfer of patients to the most appropriate care setting and to effect a reduction in the number of delayed transfers of care. The overall aim of this service is to develop and establish a trusted assessor service in Cheshire East, this service will provide a trusted assessment function through Independent Transfer of Care Coordinators. This service will initially work with existing care home residents who have been admitted to hospital and require assessment prior to transferring back to the care home. This service will in part help reduce patient length of stay as well as contributing to a reduction in Delayed Transfers of Care.
