Housing First offers secure long term general needs, self-contained accommodation with intensive support to individuals who have multiple complex needs and have experienced chronic, recurrent homelessness. Housing First offers a service with no prerequisite for demonstrating tenancy readiness and operates a harm reduction approach to substance misuse. The service offers a stable environment and personalised, service user led wrap around intensive support, with the aim of reducing levels of repeat homelessness, improving health and wellbeing and providing case management to facilitate engagement with appropriate services. The Housing First service remains with an individual even if they lose their accommodation. The service will provide assertive outreach and intensive support and case coordination to single homeless people (18+) with multiple and compound needs or high support needs who cannot be housed in existing supported housing (e.g. due to health needs). People referred to the service will have a history of rough sleeping or homelessness. The service will support people to move into and sustain accommodation. The service will ensure people are able to manage their accommodation, develop support networks and maintain their health and wellbeing. The service will offer flexible support and remain with people for as long as they need and want support. The service will aim to develop relationships with clients and build on their strengths and aspirations to support them to manage independent accommodation. The provider shall: — work with partners to ensure effective communication, co-ordination and collaboration to strengthen services provided to people with multiple and complex needs; — deliver a service that is tailored to individual needs; — deliver a service which represents good value for money for the Council; — enable service users to make empowered and positive choices about their future; — use a personalised and psychologically informed approach to support provision.
