Tender description

The London Borough of Enfield seeks expressions of interest from any suitably qualified organisations (whether they be consortia or not) for the provision of supporting people to improve their health and wellbeing and improving self-management (the service). Please see the service specification — schedule 5 — for further details. This tender is being relaunched following the suspension of the last procurement process (in March 2020, DN428200) due to Covid-19 pandemic. Interested organisations can express their interest by completing the standard selection questionnaire and uploading along with required support documents onto the e-Tendering system (www.londontenders.org). The deadline to submit is 5.00 pm on 18 December 2020. Please note that e-mailed and/or faxed submissions, whether in full or in part, will not be accepted and will result in the application being rejected. Please see other relevant documents within the London tenders portal for further information. The target groups for this service are those from the list below and who do not meet the thresholds for specialist statutory support. This service is not to replicate existing services. The focus of this service will be on: — prevention of long-term conditions – dementia, diabetes, mental health, heart disease, COPD and respiratory disease; — identification of those who would benefit from public health services – stop smoking, sexual health, alcohol and substance misuse services; — supporting people living with dementia, diabetes, COPD, frailty and anyone at risk of social isolation to manage their existing condition and improve their health and wellbeing; — linking people with other support networks for their conditions including pharmacies; — people with mental health issues being treated within primary care or at risk of developing mental health conditions, including perinatal mental health; • people at risk of social isolation; • carers of the above groups; • partnership working with London Borough of Enfield to promote and refer to existing services.
