Children and Young People Palliative Care Support in North West London

Tender description

Provision of tailored (flexible) emotional and practical support complementing clinical support for seriously ill children in North West London and their wider families and will enhance support at end of life. The service will link hospice and community palliative care services and provide better integration of care. The support provided may last from anything between a few weeks to 12 months or more, depending on a family's needs. Additional information: This service is being run as a pilot using NHS England grant funding to support palliative care enhancements.
