Tender description

The NHS Diabetes Programme, a joint partnership between NHS England and NHS Improvement, Public Health England and Diabetes UK, have awarded 10 contracts to pilot at scale delivery models for the use of total diet replacement (TDR) products alongside a 12-month package of support provided in either face to face group or one to one sessions or using digital/remote methods to eligible participants with Type 2 diabetes. Each provider will work with one of 10 ICS/STP sites delivering the intervention including 12 weeks of TDR products to around 500 participants. Delivery model: face to face — one to one Location: South Yorkshire and Bassetlaw. Delivery of an intervention to around 500 participants, which will include the provision of up to 12 weeks of total diet replacement products alongside a structured programme of support to people living with Type 2 diabetes. The package of support for this lot is to be provided in face to face one to one sessions (to those referred from primary care in line with the clinical eligibility criteria defined within the service specifications). The intervention will last 12 months providing support to ensure successful food reintroduction and weight maintenance following the initial weight loss phase. Delivery model: digital Location: Humber Coast and Vale. Delivery of an intervention to around 500 participants, which will include the provision of up to 12 weeks of total diet replacement products alongside a structured programme of support to people living with Type 2 diabetes. The package of support for this lot is to be provided using digital/remote methods (to those referred from primary care in line with the clinical eligibility criteria defined within the service specifications). The intervention will last 12 months providing support to ensure successful food reintroduction and weight maintenance following the initial weight loss phase. Delivery model: face to face — group Location: Greater Manchester. Delivery of an intervention to around 500 participants, which will include the provision of up to 12 weeks of total diet replacement products alongside a structured programme of support to people living with Type 2 diabetes. The package of support for this lot is to be provided in face to face group sessions (to those referred from primary care in line with the clinical eligibility criteria defined within the service specifications).
