Norfolk County Council wishes to award a contract to a provider to deliver services to support adults on discharge from hospital. The Home from Hospital service will help people re-adjust to living at home again, gain in confidence, and provide short term practical help after hospital discharge. The length of the support is to be no more than 6 weeks. The term of the contract is expected to for 3 years initially with the option to extend for a further 7 years. Referrals to this service will come from one of the three acute hospitals in Norfolk. Norfolk County Council, wish to engage with the market and meet with potential bidders to further understand options for delivery.
