Tender description
Norfolk County Council wishes to award a contract to a provider to deliver short term practical help for adults returning to their home after a period in hospital. The service will provide short term practical help after hospital discharge and up to 6 weeks if required, supporting both health and social care. The service will work with the three acute hospitals in Norfolk. Norfolk County Council wishes to award a contract to a provider to deliver services to support adults on discharge from hospital. The Home from Hospital service will help people re-adjust to living at home again, gain in confidence, and provide short term practical help after hospital discharge. The length of the support is to be no more than 6 weeks. The term of the contract is expected to for 3 years intitally with the option to extend for a further 7 years. Referrals to this service will come from one of the three acute hospitals in Norfolk.
