Tender description

The software solution allows the single source of truth for health and care staff by sharing the most up to date and accurate care plan for that patient across multiple health and care locations at the point of care. The solution will be developed and implemented across multiple health and care settings starting with GP practices and rolling out to other locations, namely, GP out of hours, emergency departments, acute, mental health, social care and hospices. This will initially start with addressing care plans integration on dementia and frailty in Greater Manchester locations. The solution must be capable of using HL7.FHIR resources as an interoperability standard as required by NHSX.
