Tender description
This tender is for a Short Term Residential Reablement support service, provided in registered residential care homes. The service has been established to discharge individuals from hospital, for a temporary period of up to 6 weeks, with the aim to re-able the individual to be as independent as they were prior to admission to hospital and return to their home. The service will be for residents of Coventry, predominantly people aged over 65 years, who have completed an acute episode of care but are unable, at that point, to return to their usual place of residence.
