Care and Support Services for People with Complex Mental Health Needs

Tender description

Slough Borough Council and Berkshire Healthcare Foundation Trust have commissioned care and support for adults with moderate or high complex mental health needs. The services enable service users to acquire and maintain independent living skills and to move into their own tenancy; to be focused on their recovery and rehabilitation. The services do not cover the provision of any service that includes personal care. Service users will be supported to become active citizens within the community, to access the same opportunities as their peers and to maintain links with the local community, family and friends. The Provider must adhere to the Care Act 2014, Mental Capacity Act 2005 and Mental Capacity Act 2007 when delivering the service. The Provider will support Slough Borough Council to deliver against the Five Year forward view, the Council's Five Year Plan, Health and Wellbeing Strategy 2016-2020 and the Adult Social Care (Domains 1,2,3,4) and NHS Outcomes Frameworks ( Domains 2,3,4,5).
