Tender description
Adult Community Services and Health have together identified a cohort of vulnerable people whose needs are not currently being met in the community. Individuals will have significant needs above and beyond that normally managed in residential and nursing settings. We would like to work with providers to develop a personalised service with a flexible approach, developing the service around the individual. Adult community services and health have together identified a cohort of vulnerable people whose needs are not currently being met in the community. Individuals will have significant needs above and beyond that normally managed in residential and nursing settings. We would like to work with providers to develop a personalised service with a flexible approach, developing the service around the individual. Brief descriptions of example profiles of potential residents are described below. Significant cognitive impairment, difficulty in making their own decisions/choices, even with prompting and supervision which requires constant supervision above and beyond that required in a normal residential setting. An adaptive environment is needed to manage/mitigate risk or severe distress. Psychological and emotional needs, such as mood disturbance or anxiety symptoms or periods of distress which do not readily respond to prompts and reassurance. Withdrawn from social situations and difficulty in engaging in daily activities. Behaviours of concern that pose a predictable risk to self and/or others. Compliance is variable but usually responsive to planned interventions. Behaviours which are challenging to manage, assess and plan for can include (but not be limited to), disinhibited behaviour which may distress others such as inappropriate toileting, uninhibited sexual behaviour and inappropriate interference with other service users, behaviours that require skilled management, behaviours that are disruptive to others, people who wander with purpose, young people with dementia and those of any age who are physically active. Unpredictable behaviours may at times be exhibited. Examples could include people diagnosed with neurological or other conditions affecting mental health and cognitive well-being such as Alzheimer's and other types of dementia; strokes, acquired or traumatic brain injury; Korsakoff's Syndrome or types of long term effects of drug/alcohol misuse. We would like to keep individuals as independent as possible enabling them to access community assets and have the opportunity to be connected to their community. This approach may mean that needs decrease over a period of time. In addition to the above needs some individuals may require bariatric care, have complex physical health needs i.e. hoisting, etc. inability to be cared for in the community safely or are drug/alcohol dependent. ACS works with three health systems, Ipswich and East, West Suffolk and Norfolk and Waveney. More detailed information regarding anticipated numbers and preferred locations will be made available at a later stage.
