Tender description
Adult Community Services (ACS) and Health have together identified a cohort of vulnerable people whose needs are not being met in full by the services currently available in the community. We are therefore looking to work with interested providers to deliver the most appropriate solutions. Adult Community Services (ACS) and Health have together identified a cohort of vulnerable people whose needs are not being met in full by the services currently available in the community. We are therefore looking to work with interested providers to deliver the most appropriate solutions. Whilst the people within the cohort have broadly similar needs it has been identified that two distinct groups can be evidenced. Group 1 – people with enhanced/complex care and support needs who require a time limited period in a premises-based setting to stabilise and fully assess their longer-term care and support requirements. Group 2 – people who have previously been infected by Covid-19, have been deconditioned to a significant degree and therefore as part of the recovery process require a period of reablement/rehabilitation before returning home. Ideally a successful provider would be in a position to provide both the premises and the full range of care and support services required though this is not essential and commissioners are open to the possibility of working with more than one Provider to arrive at the best solution. At the current time there would be a requirement that all services are able to accept Covid positive referrals if required. Service specifications have been drafted for both service elements but there would be scope to develop these further with the successful provider(s). These services would not work in isolation but be an integral part of a system wide solution and would work with, and be supported by, a range of social care and health professionals to ensure that a complete wrap around care and support package was in place for all service users. ACS works with 3 health systems, Ipswich and East, West Suffolk and Norfolk and Waveney. More detailed information regarding anticipated numbers and preferred locations will be made available at a later stage. Referrals into the services would be from both the community and acute settings but it is likely that most referrals will be hospital discharges. It is anticipated that no person would remain in either service longer than 6 weeks and it would be the responsibility of commissioners to ensure that an exit plan is in place for every person.
