Introduction E is a 22 year old young man with severe learning disability, autism, epilepsy and is doubly incontinent. He walks on his tip toes and is fully and independently mobile. He is largely non-verbal but is developing his use of technology and recommendations from SALT to communicate his choices and needs. He loves music, swimming, food, sitting in the conservatory, trampolining and walking outdoors. He is fully independently mobile and has a disability car for travelling around the community with his support workers. He has favoured children's toys that he uses but when frustrated he will bang his own head to express his discontent He has sensory integration issues around taste, proprioception, sound, and touch, sight which if explored and developed correctly contribute to his sense of wellbeing. His sensory needs are crucial to his well being. He has expressive eyes and enjoys being in the presence of staff who understand him and who he will approach to meet his needs. When he feels he is not getting the attention he needs, he will throw his toys on the floor to communicate his discontent. He has a close relationship with his Mum, step dad and his younger brothers who live nearby. He is waiting for a sensory assessment Support workers become very fond of E particularly workers who have known him for a long time. E can inspire affection and commitment in staff. E requires 2:1 for community access although in the past, when staff get to know E, they have taken him out for short periods on a 1:1 basis. E will respond positively to requests from staff to use his bedroom if he expresses an interest in self sexual stimulation. Brief history E was cared for by family until he entered residential education placement prior to the age of16 funded through an EHCP. He subsequently has lived with 3 supported living providers which have all failed to provide adequate standard of care to E and failed to maximise his skills and understanding of the world around him. E moved out of his last supported living placement on 1.2.20 and has been living in a respite centre whilst long term permanent accommodation and support is obtained. Family are of the view that supported living placements have demonstrated that they cannot adequately meet E’s needs and are of the view that a residential placement is the only option for E. Risks 1. Risks to health from frequent Epilepsy seizures both night and day. Frequent seizures also present a risk to E engaging regularly in social and community activities. E unable to communicate or protect himself from developing seizures. Requires seizure monitor equipment, 1:1 supervision and waking nights at all times to ensure he is positioned correctly and safely during seizures and is supported to recover naturally or with Buccal medication. E relies upon staff to collect his prescriptions and to ensure he is supported to take his medication. E is supported by staff/family to attend Epilepsy Consultant appointments. 2. There is a risk to E’s health in obtaining correct medical treatment due to his non-verbal status. If he falls or hurts himself he is unable to express the location of pain and is unable to verbalise and identify the cause of pain. 3. There is a risk that E could be financially, sexually, emotionally and physically abused by exploitative members of the public should he be unaccompanied in the company of other residents or out in the community. 4. There is risk that E may experience physical harm when out in the community due to his severe learning disability preventing him understanding the risks of crossing the road and preventing him learning how to cross the road safely. When E is being supported out in the community where there are busy roads, he accepts the use of a wheelchair to help increase his safety. E is a tall strong young man so in order to reduce the use of physical restraint in the event of dangers when crossing, E seems to accept the use of a wheelchair in this way. 5. There is a risk that E will experience a downturn in his mental health and social exclusion if he is not supported adequately to communicate and have his expression of needs and choices understood. He needs to have regular and meaningful access to his preferred activities and have access to his positive family relationships. A timetable of activities that provide him with new learning opportunities, new social relationships, a person centred approach and recognition of his adult needs will need to be put in place, actioned and reviewed. A range of social and community opportunities that bring him into contact with other young adults and carers is needed. 6. There is a risk that E could hurt children or members of the public as he has little concept of socially acceptable behaviour or the needs of others. His desire for food that he may see being eaten by others can result in him trying to forcibly take food and can result in unintentional injury to members of the pubic. 2:1 staffing whilst out in the community helps identify potential incidents and they use strategies of diversion and distraction to reduce the likelihood of harm to himself or others. Process At this point in time the Council is seeking expressions of interest only from suitable providers for supported living or residential placements as CLOSE TO CHESTER / ELLESMERE PORT area as possible. The Council will decide by way of direct award or subsequent closed tender which provider or multiple providers will be used.
