Tender description

This service covers adults (18+) who are: • ordinarily resident within the London Borough of Hounslow; • with care act eligible needs or have a service offered consistent with the S117 (Mental Health Act 2007); • adults with continuing health care needs, (registered with a Hounslow GP) who need a package of care to support them living at home; or • adults with a primary need for a period of focused rehabilitation recovery and/or recuperation, hospital discharge and/ or prevention of unnecessary admission to hospital; • people at end of life who need a package of care to support them dying at home.
