Tender description
This framework agreement has a duration of 4 years and is for permanent and short term provision of residential care home and nursing care home services for persons aged 18 years or older, within the geographical area of Stockton-on-Tees. Residential care (permanent, short stay, respite or assessment and discharge support): The provision of accommodation for persons who are aged over 18 and require personal care. Residential care (permanent, short stay or respite) for people with a functional mental health need in a residential care home setting: the provision of accommodation for persons who are aged over 18 and require personal care. Residential care (dementia) (permanent, short stay or respite): the provision of accommodation for persons who are aged over 18 and require personal care with assessed additional needs. Residential care (dementia) (permanent, short stay or respite) for people with a functional mental health Need in a residential care home setting: The provision of accommodation for persons who are aged over 18 and require personal care with assessed additional needs. Nursing care (permanent, short stay, respite or and discharge support): the provision of accommodation for persons who are aged over 18 and require nursing care. Nursing care (dementia) (permanent, short stay or respite): the provision of accommodation for persons who are aged over 18 and require nursing care. Short-term interim rehabilitation beds: short term (up to 6 weeks) funded placement to support rehabilitation to regain independence with an aim to return to their own homes as soon as possible. These beds are part of the winter resilience plan. Bariatric care: services within adult bariatric population with a physical disability but should also be made available to other adults where their short-term needs can be facilitated within these settings and there is supply available 6. All standard expectations (all conditions up to and including Appendix 12) in this specification plus additional expectations in Appendix 15. Functional mental health: the provision of accommodation for persons who have a mental health need to prevent deterioration, delay dependency and support recovery and commits to improving their health and well-being.
