Tender description
We are asking for expressions of interest from organisations/individuals who would like to work with us for four months, July to October 2020, to undertake a review of the step down/ discharge to assess initiatives. This would mainly comprise: • Qualitative interviews with key stakeholders, including people who were placed in the units and their families/carers, care home staff, commissioners, hospital discharge teams (health and adult social care), community health team, hospital staff and primary care and any other stakeholders as relevant. • Benchmarking / good practice from similar models in other areas to inform wider context.
