Tender description
The post diagnosis support service will be for adults with dementia, their families and carers who have been identified and assessed as requiring support. This includes people under 65 who are employed and people over 65 at the Early State (Moderate Decline) to Mid-Stage (Moderately Severe decline) who may transition to statutory specialist provision. The Council sought tenders for the provision of dementia support services for people post diagnosis and also for Early Onset.
