Intermediate care, for the purpose of this service, provides short term assessment, rehabilitation, re-ablement and recovery care, delivered in partnership with local health and social care organisations, to support people to improve or regain independence, health and wellbeing. It provides an alternative to admissions into acute or long term care and is used to facilitate the transfer of care from hospital, thus aiming to support people to return to their own homes as soon as possible. The service will provide individualised packages of care that responds to the need of the resident, their quality of life and allows them to make choices about their future, based on an assessment of need which identifies the health and social support that each resident requires, including the therapy input which will be provided by RIACT (Responsive Integrated Assessment Care Team) services in Darlington to each resident, but supported by staff within the Home setting.
