Tender description

The contracting authority seeks suitably experienced and qualified providers for the provision of Care at Home and in the Community for Children and Young People. The council anticipates this contract for services to operate as a framework agreement for a period of 4 years. This service will provide care and support to children under 18, who have been assessed to meet the council’s eligibility criteria for unmet needs. This includes (but is not limited to) children who have assessed care and support needs associated with physical ill health, learning disability, physical disability, and communication impairments. The primary objective aims of the service are to enable children and young people to: — live as independently as possible within the family environment, — develop and achieve skills, — maintain their own networks of support within the community, — access meaningful community activities which promote learning and development: — have opportunities for socialising, — be safe within their home and community environment, — have opportunities for choice and decision making, — achieve reasonable outcomes as identified in their Care and Support Plan.
