Tender description
Kent Community NHS Health Foundation Trust (KCHFT) is seeking tenders from suitably experienced providers who can deliver a health and social care village service. The health and social care village service supports the discharge of patients who have been assessed in an acute hospital setting as safe to be discharged from an acute hospital environment and into a nursing home. Here, they will receive a further assessment of need, rehabilitation and a period of recovery while developing a plan of care and onward referral if required. This service is part of the East Kent KCHFT portfolio of Services which seek to improve delayed transfer of care and reduce hospital admissions/readmissions. KCHFT is looking to work with social care providers, who are registered with the Care Quality Commission (CQC) for the provision of residential nursing care, to support the identified care needs as part of the discharge plan of care for the patient. The Trust is seeking bids to enter into a framework contract, under which predominantly block bed arrangements will be commissioned. The purpose of this is to generate and secure dedicated KCHFT capacity while offering a surety of revenue to providers. Providers have flexibility to bid a range of options for the Trust to consider. Details are given within the tender pack. The contract will run until the end of March 2021, unless terminated in accordance with the terms and conditions. The Trust may extend the contract term twice by 12 months each (to March 2022 and March 2023) at its sole discretion. Suppliers should note, this framework agreement is anticipated to reopen to new entrants at regular intervals of not less than each 3 months. This is to support suppliers who may have failed on their first application, with Suppliers getting two attempts. However, suppliers should note it is anticipated block bed arrangements will be called off the framework contract from its inception. Expressions of interest, clarification questions and other communication will only be accepted via the Trust Procurement Portal.
