Health care services in Jersey are undergoing transformation. A series of reviews and consultations has identified that the current system is not sustainable as the population of Jersey ages. Health care has centred around the hospital and community practitioners, such as GPs, nurses and pharmacists have been underutilised. The new care model seeks to put patients and families at the heart of the system, delivering care closer to home, using multi-disciplinary teams. Over the next few years the delivery of some health and social care services will move out of the hospital and into the community to be provided by GP practices and other agencies, enabling the hospital to focus on specialist and emergency care, intensive care and maternity services. To make these changes the Health and Community Services Department will have to work differently with Jersey's current community providers, including GPs. To enable this transformation the Health and Community Services department (HCS) are seeking to create a General Medical Services contract with General Practice, which will be the first of its kind in Jersey.
