Various specialist post stroke services provided within Islington. — communication services: aphasia support integrated into Islington’s stroke pathway. Operating from the Outlook centre and providing post stroke communication interventions to Islington residents, — navigator services: post stroke navigation support to Islington residents, integrated into Islington’s stroke pathway. This service will link with HASU to support recovery including SNAPP outcome indicators and reviews, — advocacy and outreach services: further post-stroke support working on information and prevention; integrated into Islington’s pathway and operating from Manor Gardens. Islington provides the following stroke support services as part of an integrated pathway: — communication services: aphasia support integrated into Islington’s stroke pathway. The above services are being extended for 1 year (19/20) via this VEAT notice and further development will be completed in 19/20.
