Tender description
Nottinghamshire County Council is seeking tenders from Providers of Residential Care Home services for adults, to join a Framework Agreement for the provision of Short Term Assessment and Reablement Beds for Older Adults with Dementia and/or Physical Frailty. The Framework will be available for any relevant public body to utilise within Nottinghamshire It will be established for an initial term to 20.1.2021, with an option to extend for a further period of up to 2 years. This project also includes the initial call-off by Nottinghamshire County Council of block funded care home beds under 2 Lots. Providers must be successful in their bid to join the Framework in order to be considered for award of a call-off lot contract. A Bidder Information Event will take place at 14:00 on 13.3.2019 at County Hall, Nottingham NG2 7QP. Please note; a further opportunity/opportunities for organisations to bid for inclusion in this framework may be advertised. The Service is aimed at people who are at risk of needing long term care in a care home following a stay in hospital, or to prevent unnecessary admission to long term care or hospital following a crisis at home. The purpose of the Service is to provide an environment outside of hospital, or their own home, where people receive a more thorough assessment by a multi-disciplinary team and a short period of reablement, before any decision about long term care is made. The maximum length of stay for an individual is expected to be up to three weeks, but this may vary, depending on initial assessment. Beds will primarily be used for adults aged 65 years and over. This will include people with dementia, who may exhibit behaviours that challenge service delivery; and/or physical frailty. The beds may also be used for younger adults with long term conditions and/or physical frailty. The care home accommodation must be appropriate for the needs of the Individual and safe for people with cognitive impairment and/or dementia as well as people with physical frailty. Individuals must have access to basic kitchen facilities. Two initial call off contracts each for a block of 5 beds in any of the Broxtowe, Gedling or Rushcliffe Borough areas of Nottinghamshire are included in this tender.
