Tender description

Bromley CCG is seeking to appoint an organisation for the provision of a service delivering community cardiac rehabilitation and exercise referral programmes. The community Cardiac Rehabilitation Programme is to support a hospital based phase 3 and deliver phase 4 of an accredited BACPR approved cardiac rehabilitation programme across Bromley. This programme is for patients who have either had a recent cardiac event or require ongoing cardiac support for an identified heart condition by primary care. Included in this contract will be an Exercise Referral Programme to support patients with long term conditions. This will be delivered in the areas of Orpington, Beckenham and Penge.
