Tender description

The City's Department of Community and Children’s Services (DCCS) has prioritised the provision of efficient and effective person centred, integrated health and social care services within its business plan and its joint health and wellbeing strategy. To this effect the City are procuring the services of experienced providers of early intervention and prevention services. Needs analysis, service gap identification, and market engagement have identified that for DCCS to deliver an improved, integrated and improved outcomes — based early intervention/preventative offer to adults the department should put in place a coordinated supplier infrastructure which will move the department towards developing a system approach with consistent and interlinked outcomes which will further promote independence, delay or reduce the development of health and social care needs and reduce hospital admissions. The successful provider will be required to coordinate, communicate and connect services together and to deliver directly, or by way of subcontracting, a range of community support activities and interventions that can be offered to people within the community. Coordination — assessing, recording, monitoring and reporting on outcomes and for people using services in the community, including those from other providers, — collecting, recording and reporting usage and performance data from other services in the community, including those from providers of community support function activities, — collecting, recording and reporting signposting and referrals into and out of the City Community Connections service, — arranging and participating in service development and review meetings including hosting the City Outcomes Delivery Co-Production Board. Communication — develop and maintain a website to provide a central communication point where information about community based activities can be easily accessed and shared and contain a specific section with support and information available to carers, including young carers and reference to the FYI website for families and young people’s information service and City Corporation’s website pages, — provide a telephone number (with voicemail function) that can be promoted as a central “one stop shop” for information and enquiries about support and activities that are available, — develop and distributing a range of communication materials to promote the City Community Connections service, — promote the service to a wide range of statutory and community organisations or leaders who are likely to make referrals into the service or who can provide support and activities to people. Connection — offering information about local community activities that may meet a person’s needs, — ensuring all people who require an individual befriender or volunteer to support them are matched with one, — offering an individual, the option of a full community support assessment through the service or alternatively a referral to their social prescribing coordinator based in a GP practice. The budget for City Community Connections Services will be a maximum of 200 000 GBP per annum. The duration of the contract is 3 years, subject to the right of the City (at its sole discretion) to exercise its right to extend the contract by up to 2 further periods of 12 months. The maximum length of the contract is therefore 5 years. There are 2 main functions of the service: self-directed support, and Appointeeships. Self-directed support function will include: — information, advice and guidance, — support planning, — employment information, guidance and support, — direct payment support, — provide expert advice to the City of London Corporation Adult Social Care Team and members of the public on self-directed support and any emerging developments in this area. Appointeeship function aims to facilitate financial safeguarding of individuals with limited capacity to manage their own financial affairs. This will include: — making applications to the DWP, — providing a money management service to individuals concerned, — facilitating bill and service payments, — ensuring all relevant benefits are claimed and manage any benefits, — budget and debt management, — contributing to needs reviews and safeguarding reviews as required. The budget for City Finance Services will be a maximum of 30 000 GBP per annum. The duration of the contract is 3 years, subject to the right of the City (at its sole discretion) to exercise its right to extend the contract by up to 2 further periods of 12 months. The maximum length of the contract is therefore 5 years.
