Tender description

This procurement intends to bring together a number of services which are currently separate together under a single contract. The service will meet the following needs for those with dementia and/or their carers/families: — provide a highly responsive, individualised information and signposting service, — provide ongoing support, information and guidance; helping to maintain patient independence, improve their sense of well-being, and gain more control of their lives, — to assist with identification of needs and increase access to services, — via the Dementia Cafés and Groups, increase the opportunity to access post diagnostic support closer to home. It is expected that TUPE will apply. The contract duration shall be 3 years with the option to extend for a further 2 years in 12 month increments at the sole discretion of the commissioner. This procurement is not divided into lots.
