Tender description

The Authority is seeking to appoint a provider to manage and deliver the NHS Health Checks programme to increase the number of health checks offered in Darlington and the uptake. The appointed supplier shall remain completely neutral and when appointed will not provide the NHS Health Checks themselves and remain neutral when dealing with GP Practices. The supplier must sub contract with each of the 11 GP Practices in Darlington for the delivery of NHS Health Checks to each GP patient list in accordance with the Contract and is responsible as main Contractor to the Authority for the services. The supplier must be the interface between the Authority and GP Practices and visit individual GP Practices to provide support and advice where necessary.
