Tender description
To commission a managed and coordinated Targeted Aiming High Short Breaks Service for children and young people with a disability up to their 18th birthday, and their families in Stoke-on-Trent. This service will be managed by 1 single Lead Provider who, through a series of sub contracts, will engage, collaborate and work with other organisations to deliver a broad range of short break activities meeting the needs of our families and their children/young people.
