Tender description

The service will be about linking people up to activities in the community which they may benefit from and connecting people to non-medical and universal statutory sources of support. It will also include signposting and bridging, which will support the person to overcome barriers which are stopping them from accessing community support, whether that is a practical (e.g. transport) or an emotional barrier. The service should also support better health awareness and self-management and include the use of technology to enhance these deliverables.
