The overarching purpose/output of this project is to develop a strategy for personalisation for Tower Hamlets for the next 3-5 years. The overarching purpose/output of this project is to develop a strategy for personalisation for Tower Hamlets for the next 3-5 years. This strategy will need to include and/or be informed by: 1) A mapping/consolidation exercise of work that has happened to date; 2) A quantitative and qualitative evaluation of the work that has happened to date; 3) A vision for personalisation for Tower Hamlets, which has been developed and agreed with all key partners in the system; 4) Priority areas of focus for future years, working towards the vision that has been agreed; 5) Details on the level of resource that may be required to deliver the strategy going forward, for example, in terms of programme management costs; 6) Details on how PHBs can be funded on a sustainable basis going forward, for example, through extracting money from existing contracts; 7) A delivery plan, to accompany the strategic objectives; 8) Measures to track progress of the strategy, once implementation has commenced; 9) Some outline modelling of the potential for the programme to improve health and well-being outcomes and/or reduce pressure on other parts of the system e.g. through a reduction in primary and secondary care utilisation.
