Dementia Support Service on behalf of NHS Wandsworth CCG

Invitations to tender are now welcome from suitably experienced and capable providers to deliver the provision of a Dementia Support Service. WCCG has developed a new service model which will play a central role in providing systematic and proactive support to people and their carers/family who are living in the community with dementia through personalised case management. This new service will work in close collaboration with health and social care and the voluntary sector and be a single point of access linking all dementia support in the borough. The Dementia Support Service will support people diagnosed with dementia, and also their carers, families and those close to them to seek help, advice and support at any stage of the illness and will provide a single identifiable point of contact.
