Tender description
Somerset County Council is intending to establish a light open framework agreement under which to procure regulated short term reablement services via the Proactis Portal. The total value of the framework over the agreement period for all services is 4 000 000 GBP. We are seeking interest from organisations that would like to join the framework and provide these services in Somerset. The emphasis of short term reablement is to work with appropriate agencies to support a person to maximise their independence and reduce or overcome the need for longer term funded support, by working with the rapists, social workers and community agents to support successful reablement and help in the assessment of the right support at the right time. The outcome will be helping people manage themselves, so that where possible, needs are reduced. Short term reablement allows for good interaction between professionals and will identify local community based and voluntary resources to aid person centred planning and shared decision making. This will build greater resourcefulness within communities with a vision for self-care, prevention and active engagement. The reablement service will support people in their place of residence and is short term, and for a fixed period. It will focus on stabilising care plans, designing well-coordinated person centred plans, and support smooth transfers to other appropriate services. The key functions: Support people being discharged from community hospitals or people who may be at home and to: — deliver and monitor a reablement care and support service that is regulated to provide personal care, — liaise closely with health and social care professionals, as well as new and existing services, — adjust and stabilise support and source and arrange for appropriate support, — arrange for assessed care and support requirements which complement the care plan and focus on a reducing provision, — crisis/urgent response provision, — stabilise a person to a point where they are able to manage themselves independently or arrange for a smooth transfer to longer term support services.
