Tender description
To provide Radio Frequency Segmental Ablation Catheters for vein ablation. This is to include the EndoThermal Ablation Procedure Pack, Generator, Tumescent pump and foot pedal (if required). Lot 21700: VENUS CATHETERS AND ASSOCIATED MEDICAL DEVICES Stage 1 – Supplier Questionnaire (SQ) • Please complete the Supplier Questionnaire and upload it with any supporting information before the Pre-qualification deadline. • The Invitation to Tender (ITT), offer schedule and T&C's documents are for information only at this stage. Please do not complete and upload them with your SQ. • Following evaluation of the SQ’s short-listed suppliers with be invited to complete a full tender submission at Stage 2 - Invitation To Tender.
