Tender description
The aim of the Service is to facilitate a speedy return home from hospital by providing a domiciliary package of care for up to six weeks to enable a full assessment of need to take place in the Service User's home environment. The Service will need to be provided seven (7) days per week, 365 days per year. This is being procured under the Light Touch Regime.
