Tender description

The service specification focuses on core service standards, detailed list of requirements, organisational capabilities and the expected outcomes for customers. The specification will require potential providers to be familiar with the principal legislation and statutory requirements relating to the provision of the following services: 1) Independent mental health advocacy; 2) Independent mental capacity advocacy; 3) Care act advocacy; 4) Carers support service; 5) Social isolation preventative service. Our intention is to commission a gateway to support services hub, which will provide a joined up, seamless approach to delivering preventative social care services and statutory advocacy support, promoting community capacity and reducing demand on Social Care and Health Services. People will be able to access the service themselves (self-referral), or be referred by a professional such as their GP or social worker. The hub model will deliver: — a single access point for all referrals to Gateway services, which will be triaged to determine the best way to support the individual, — a triage process that will determine the pathway and priority of each referral or enquiry. The benefit for service users is that they will only need to complete one Gateway screening, and not be subjected to a screening multiple times, should they need a different service or re-access services in the future, — the Hub will not provide generic advice, but will develop working relationships with other services (such as CAB) that provide information, advice and guidance, — the provider will be expected to deliver a time-limited case work to people accessing the service. This would involve an initial: —— specific advice or advocacy depending on which element of the service they require, —— direct-delivery of case work, —— follow-up for a specific period of time, — a service that requires clients to only speak to 1 provider regardless of needs, rather than having to seek support from multiple services.
