Tender description

The London Borough of Merton (“the Council”) is inviting tenders to provide housing related support to residents in the borough. This tender is being offered as 5 lots: — Lot 1: Mental Health — Lot 2: Floating Support — Lot 3: Homelessness Prevention – Young People at Risk — Lot 4: Homelessness Prevention (Adults) — Accommodation Service — Lot 5: Homelessness Prevention (Adults) — 24-hour Direct Access Hostel The service provider is required to deliver HRS accommodation based support services to 23 service users with mental health conditions at any one time. This support will be offered to service users in an established supported housing scheme. The service will: — maximise the ability of service users with mental health and other complex needs, including substance misuse issues to live independently within the community, — ensure effective coordination between health and community support agencies in delivering a comprehensive support framework for service users, — enable service users to identify and make appropriate health, safety and lifestyle choices to maximise their well-being, — support service users to develop their independent living skills to maintain independence in their homes and reduce the risk of breakdown of tenancy and homelessness. The service provider will: — maximise the ability of service users to live independently within the community, — support service users to manage their health and well being, and communicate successfully with other agencies when accessing appropriate services, — support service users to maintain independence in their homes and reduce the risk of breakdown of tenancy and homelessness, — support service users to develop maintain money management skills, — enable service users to identify and make appropriate health, safety and lifestyle choices to maximise their well being and independence. The scope of the service will require the service provider to: — maximise the ability of service users to live independently within the community, — support service users to manage their health and well being, and communicate successfully with other agencies when accessing appropriate services, — support service users to maintain independence in their homes and reduce the risk of breakdown of tenancy and homelessness, — support service users to develop maintain money management skills, — enable service users to identify and make appropriate health, safety and lifestyle choices to maximise their well being and independence. The service provider will: Build and maintain effective relationships with a range of partners to enable service users’ mental and physical health needs to be met within the support plan delivery. Ensure support networks are established to enable service users to identify and make appropriate health, safety and lifestyle choices to maximise success in their transition to settled accommodation and independence.
