Social work services with accommodation

Tender description

The service will provide a short term and emergency accommodation based Housing Related Support (HRS) service for Warwickshire young people aged 16-25 in order to prevent, reduce and/or delay their need for care and/or support in the longer term. There is a cohort of vulnerable young people and young parents in Warwickshire who for a variety of reasons lack the skills needed to be able to live independently who would benefit from accommodation provided for a temporary period where they can be supported to gain those skills and develop resilience. Young people aged 16-25 including care leavers and young parents who are resident in Warwickshire, homeless or at risk of homelessness and assessed as being on the ‘Edge of Care’. This service will give them the skills to move on to their own longer term accommodation where they can live more independently, safely and healthily in the future, thereby preventing homelessness and the need for more intensive support and/or health care. The aim of the service is to provide a short term and emergency accommodation based Housing Related Support (HRS) service for Warwickshire young people aged 16-25 in order to prevent, reduce and/or delay their need for care and/or support in the longer term. The service will be in North and Central Warwickshire which comprises; North Warwickshire, Nuneaton and Bedworth and Rugby. The service will be in the South of the County which comprises; Warwick and Stratford-upon-Avon
