Tender description

NHS HMR CCG and Rochdale Borough Council are proposing a new integrated service model for children’s community health services that will support our drive to improve outcomes for children and young people. The service delivery model will build upon the existing integrated children’s acute and ongoing needs service (CAONS) and ensure that prevention and early help health services are also included. The service model at this stage will include the following existing services: — children’s acute and ongoing needs (CAONS), — 0-5 universal public health services (health visiting), — 5-19 universal public health services (school nursing). The service will operate as a single entity, with each individual service element contributing to and supporting delivery of a shared set of outcomes. The service delivery model will build upon the existing integrated children’s acute and ongoing needs service (CAONS) and ensure that prevention and early help health services are also included. The service model at this stage will include the following existing services: — children’s acute and ongoing needs (CAONS), — 0-5 universal public health services (health visiting), — 5-19 universal public health services (school nursing) The services are to be delivered as part of a comprehensive 0-19 years (0-25 years to meet the CCG’s statutory obligations regarding specialist educational needs and disability (SEND)) seamless service for children and young people.
