Supported Living Services for Adults

Tender description

Medway Council is seeking tenders from suitably experienced providers that can deliver Supported Living Services to support adults from age 18 with Learning Disabilities, Mental Health Needs, Sensory Impairment, Physical Disabilities, Autistic Spectrum Conditions, and People with Multiple and Complex Needs including supporting people’s personal care needs.  The Provider should support Service Users to develop new skills, regain lost skills through a re-ablement approach and promote independence, reducing their need for assistance. Packages of support may vary from a couple of hours a week to support provided up to 24 hours a day, 7 days a week and shall be provided in the home environment or in the community and include supporting personal care needs.
