Intermediate care is a range of integrated services to promote faster recovery from illness, prevent unnecessary acute hospital admission and premature admission to short and long-term residential care, support timely discharge from hospital and maximise independent living. The aim of the Residential Intermediate Care service is to provide high quality, person-centred, short term intermediate care to residents from their own home as step up and to those being discharged from hospital as step down. The service will be considered for all Blackburn with Darwen residents who have suffered an acute trauma or illness which has resulted in a need for therapy and where this cannot be delivered in a person’s own home. The service will empower service users to recover from episodes of ill health and to return home through the provision of individualised support and therapy, care and wellbeing, medical oversight and nursing, where applicable. The service is designed to optimise an individual’s level of independence based on identified needs and in accordance with the agreed care plans. Keywords: Intermediate Care
