Homelessness Health and Wellbing Coordination Project

Tender description

The Project is now to be expanded across all WCC commissioned supported housing pathways with a wider brief of health, wellbeing and supporting good interfaces within the pathways. The commissioned pathways include: Rough sleeping pathway (from outreach to supported housing) Mental health pathway Young adults pathway With further bespoke services dedicated to accommodation projects relating to women, domestic abuse and the prevention of recidivism Each pathway will need attention individually, but many initiatives will be common to all. The overall aims of the service will be to: -Raise the capacity of providers of hostels, supported housing and outreach to support service users to improve their health through training, information, initiatives to improve access to health services and by providing a range of health events -Improve access to primary health services for people receiving support services in the borough by attending to interface issues and reducing unplanned A&E admissions and ambulance call outs. -Improve awareness amongst statutory health providers around the challenges clients across commissioned services face accessing health services and the needs of these client group -Using intelligence gained through the enhanced coordination, influencing service provision in the voluntary and statutory sector to improve outcomes
