Tender description

The London Borough of Havering are seeking expressions of interest from service providers to provide a Supported Hospital Discharge service across 3 London Boroughs; Barking and Dagenham, Havering and Redbridge. This procurement will take place in Oct/Nov 2019. The new contract will commence in April 2019. The value of the contract is in the region 280 000 GBP — 300 000 GBP per annum. The contract period will be 3 + 2 years. The supported discharge service provides support to vulnerable adults as they are discharged from hospital back into the community. The service helps to reduce delays in discharge by providing short term, practical and emotional support to facilitate a smooth and timely discharge from hospital. This enables people to return safety to their own home and avoid unscheduled re-admissions. This is particularly important to those people who live alone who may otherwise be unable to return home and live independently. The services that are designed are preventative and support statutory services but do not replace them, fully or partially.
