KMCAS-129 Contract for Stroke Support

Tender description

Tenders are invited by The Council of the Borough of Kirklees (the "Council") from Service Providers/Contractors with relevant experience and ability to demonstrate sufficient capacity for Kirklees Stroke Information, Support & Targeted Prevention; Six month Reviews following Hospital Discharge; and Communication Support (the "Service"). It is anticipated that is to commence on the 1st October 2019 and will expire on 30th September 2023 (2+2 (4 years). The Tender Document comprises the Instructions for Tendering, Form of Tender, Conditions of Contract, General Specification, Selection Questionnaire and sets out the nature and extent of the requirement and the conditions upon which the Services are to be provided. The total contract value is £136,000 per annum. Tenderers are invited to submit prices for the Service as specified in the Pricing Schedule.
