Conwy County Borough Council wishes to appoint a suitable, qualified and experienced domiciliary care provider to deliver supported living services to 2 individuals with disabilities (male and female) living on their own in their own flats within the same premises in Llandudno. Conwy County Borough Council wishes to appoint a suitable, qualified and experienced domiciliary care provider to deliver supported living services to 2 individuals with disabilities (male and female) living on their own in their own flats within the same premises in Llandudno. The service provider must have the necessary expertise and specialism in order to provide proactive, creative, person centred care to enable them to live active, healthy and safe lives. The contract awarded will be for the duration of 3 years with an extension for up to 2 years. The service is required to commence immediately following contract award in accordance with the timescales specified in the service provider’s tender proposal.
