Commissioners within NHS Brighton and Hove CCG are seeking views of healthcare providers for a home to settle service. The anticipated value of the service is 300 000 GBP - 400 000 GBP a year. The commissioner is considering a contract period of 3 years plus an option (option of the commissioner) to extend the contract by up to a further 24 months. The requirement is for a home to settle service for patients attending, or being discharged from Royal Sussex County Hospital, Brighton. Home to Settle Service (Reference 19.513) NHS Brighton and Hove Clinical Commissioning Group Request for Information (RFI). The commissioner is seeking the views of healthcare providers for a home to settle service. The anticipated value of the service is 300 000 GBP - 400 000 GBP a year. The commissioner is considering a contract period of 3 years plus an option (option of the commissioner) to extend the contract by up to a further 24 months. The requirement is for a home to settle service for patients attending, or being discharged from Royal Sussex County Hospital, Brighton. The aims of the home to settle service are: To support patients who live alone or with a carer, to return home safely from hospital following A&E attendance or ward discharge. To work proactively with discharge teams and other teams such as Accident and Emergency to support timely discharge from, or avoid admission to RSCH. To carry out an initial risk assessment upon return home and agree any additional referrals/signposting as appropriate to ensure patients remain independent and rebuild their confidence. To work collaboratively with key stakeholders to ensure patients are referred into appropriate services. The service is a key element of health and social care discharge pathways and will receive referrals from discharge teams, wards and Accident and Emergency staff following patient needs assessments and suitability for discharge and will deliver the service through: — facilitating safe and timely discharge or admission avoidance, — providing safe car transport home, — providing initial settling-in support in the home, — signposting/referrals to ongoing help (where needed/wanted and consent given) to the patient and/or carer, — undertaking an initial risk assessment in the home to reduce the risk of falls/further hospital admission to the patient, — ensuring vulnerable people are settled back in at home: deliver, set-up, explain aids and equipment as appropriate, — providing up to one additional face-to-face visit and up to 2 telephone calls post discharge and referrals to health, social care, housing, fire and rescue and/or other services where required, — practical support: light shopping/housework; snack preparation; disposal of perishable foods/other household rubbish; assist with bedding; check/turn on heating; assist with paperwork, etc. — listening to the patient and their needs and concerns to inform referral, — signposting or referrals to other services, — help to access local activities/support, — carrying a selection of stock food items and clothing for those patients who require it, — where a patient is not able to self-transfer into the car, the provider will work with patient transport to offer the support element of the service once they arrive home, — attending Board Rounds, MADE events and other discharge meetings to identify patients suitable and discharge date, — initial assessment on the ward with further support planning once at home, — presence at A&E and the acute floor,. Service outcomes: — patients are provided with reassuring, safe and companionable car transport home from hospital, — patients are supported to return to their home safely with their immediate needs met, — patients receive the information, advice and support they need in order to prevent unplanned readmissions, — patients maintain/regain their independence and rebuild confidence, — patients are signposted to/integrated into services in the wider community, — reduce discharge delays, — reduce admissions for patients whose needs are more social than medical. This RFI process is designed to help the commissioner form a view of the best way to commission the service and is not the beginning of a tender exercise.
