Tender description

NHS Wandsworth CCG is looking to appoint a suitably qualified and experienced provider to co-ordinate and deliver a social prescribing service across the Borough. The social prescribing Service will: — improve social and clinical outcomes for people with long-term conditions and their carers, — make more cost-effective use of NHS and social care resources, — enable clinicians to refer patients to a range of local, non-clinical services to improve their health and well-being, — address people's needs in a holistic way and facilitate access to the right place, first time, — contribute to building a more supportive, inclusive and resilient community. NHS Wandsworth CCG is inviting expressions of interest and bid responses from suitably qualified and experienced providers to co-ordinate and deliver a new social prescribing service across the Borough. The social prescribing service will: — improve social and clinical outcomes for people with long-term conditions and their carers, — make more cost-effective use of NHS and social care resources, — enable clinicians to refer patients to a range of local, non-clinical services to improve their health and well-being, — address people's needs in a holistic way and facilitate access to the right place, first time, — contribute to building a more supportive, inclusive and resilient community. Further details are provided within the service specification and Memorandum of Information (MOI). The contract will be for a minimum of 12 months with the potential to be extended for a further 12 months (up to 2 years maximum), dependent on local funding sources being confirmed for year 2. It is expected that the contract will be awarded by the end of September 2019 to enable the new service to be mobilised asap, ideally from November 2019. The procurement process is a bespoke competitive process involving a single stage of application, and being run in line with the requirements for a service falling within scope of Schedule 3 of the EU Procurement Directives 2014.
