Doncaster CCG wishes to appoint frameworks of providers who can provide consistently high quality, home based complex care services to adults aged 18 years and children who have been assessed as eligible to receive continuing healthcare (CHC). A framework of providers to deliver a continuing healthcare complex home care service for adults and children and young people with specialist needs and/or a disability.
