Tender description

The Service is required to provide our clients with a range of practical and emotional support to assist with one off tasks as well as those more complex cases spanning between 4-6 weeks. Contact clients either face to face or via telephone. The service will be delivered to Adults only over the age of 18 years, but it is anticipated that the majority of referrals will be for people aged 65 and above who are socially isolated or have other vulnerabilities. They will work closely other services commissioned to support the hospital to ensure that all patients have the high level of care and support expected from Whipps Cross upon discharge. Objective criteria for choosing the limited number of candidates: The potential provider is requested to submit evidence and statements in relation to all the below requirements when submitting expression of interest. Please ensure that the submission to the below points are limited to max. 2000 words in total (not per question). The potential candidate will be shortlisted for competition as follows: 1. Will be able to demonstrate experience in delivering a similar project/s in a healthcare environments;. 2. Will have at least performed one similar project within the NHS environment relevant to Bart's requirement; 3. Have the capacity to deliver the proposed scope of contract and a team with suitable skills to ensure achievement of delivery timescales? 4. How you will provide an account management approach for delivery of a contract that ensures excellence in delivery of key contract outcomes being sought by Bart's? 5. Confirmation of compliance with National Service Framework for Older People: Standards 1, 2, 3, 6 and 8. 6. Confirmation of compliance with the following Safeguarding Standards: • Compliance with Working Together to Safeguard Children document March 2010 • Compliance with London Safeguarding Children’s Board Child Protection Procedures 2007 • Compliance with Care Quality Commission Regulations on Cooperating with other providers: Outcome 6 (CQC Quality and Safety Standards 2010) • Compliance with Care Quality Commission Regulations on Safeguarding and Safety: Outcome 7(CQC Quality and Safety Standards 2010) • Compliance with Section 11 Children’s Act 2004 • Training all staff to levels 1, 2 or 3 according to the level of contact they have with children and in line with Working Together to Safeguard Children document March 2010. • Keeping a robust, updated database system for staff attendance to safeguarding and child protection training at each level. • Ensuring that all staff in contact or working with children have been DBS (Disclosure Barring Service) checked and registered with the Independent Safeguarding Authority (ISA for Vetting and Barring). • Ensuring staff who have regular contact with children and families receive appropriate safeguarding supervision and that the organisation has an updated Supervision Policy that is reviewed regularly. • That the organisation complies with any investigations undertaken, e.g.; Sudden Unexplained Incident (SUI),Serious Case Review (SCR) Keywords: DN385197, Hospital to Home
