Tender description

Provision of hospice inpatient, outpatient and community care for Barnet patients. The aim of this service is to provide specialist palliative care to adults with life limiting illness in Barnet in support of the multi-disciplinary approach to palliative and end of life services, working collaboratively with primary care clinicians, locality integrated teams and other providers to enable patients with specialist palliative care needs to maintain their well-being and independence, whilst also supporting their family, friends and carers’ during the changing phases of their illness. It is essential that this service is run in a location that is convenient for Barnet patients and allows their family members to attend easily on a daily basis. Therefore it must be able provide inpatient care within 30 minutes journey from Barnet from the start of this contract period onwards.
