Rough Sleeper Outreach and Resettlement Service

Tender description

The aim of this service is to prevent and reduce rough sleeping and homelessness in Cornwall, reduce unplanned use of emergency services and enhance quality of life for people with health and wellbeing needs. The key outcomes of the service for individuals are to result in improve self-management of health and wellbeing and increased independent living skills, with a focus on securing accommodation. This will be achieved through the delivery of three service components including the prevention of rough sleeping/homelessness, street outreach for rough sleepers, and move on and resettlement support.
